HOW THE FCAHV OPERATES

An individual joins the FCAHV through application and payment of a one-time membership fee.  The membership includes dependent, minor children. 

MEMBERSHIP FEE

Our FCAHV's lifetime membership fee is $25 for each adult member (21 years and older).  The fee is used to cover the cost of the Member's Packet, Newsletters, and ongoing operating expenses. A $15 fee for each adult is charged when members transfer from another affiliate within the United States.  There are no further dues.

The member receives the Membership Packet of instruction and information that includes, but is not limited to . . .

● Before I Go You Should Know 

 A booklet that provides a vehicle to record and organize pertinent information. For example . . .

• Advance Directive / Living Will / Health Care Proxy / Power of Attorney

(Names and addresses of individuals involved with each document)

• Designated Agent for Body Deposition

• Funeral & Memorial Wishes

• Information for Obituary

• Memorial Contributions/Information for Death Certificate

● Funeral Arrangement Form

● Funeral Planning Guidelines

Once you have completed your plans the Funeral Arrangement Form, you received in the packet, should be completed and copies distributed to (a) the person appointed to control the disposition of your remains and/or your next of kin, (b) the chosen Funeral Home, and (c) FCAHV.

Upon death, the family or representative will notify the Funeral Home for execution of the designated services.
